Obtaining access to CSM's Banner System
All CSM employees will be required to submit Banner approval forms in order to access the Banner system. Please complete the following forms, and have them signed by the appropriate personnel in your department.
Please note: you do not need to submit these forms if you have already done so during ePrint training.
- Banner General Access Form
- Banner Finance/Student AR System
- Banner Student System
- ODS Authorization Form (word document)
- Pre-Employment Authorization Form
Note that by signing the access forms (or by accessing Self-Service Banner), you understand and agree to comply with the CSM Computing & Networking Resource and Responsible Use Policy & Guidelines and the CSM Administrative Data Access Policy.
