Health Risk Assessment (HRA)
We encourage you to take the HRA online through the Anthem web site. Completion of the HRA is private and confidential. Your specific information is not shared with Mines, CHEIBA, or any other persons.
To take the HRA, complete the following steps:
1. Go to www.anthem.com
2. Click on the member login box
3. Click on New User if not currently registered
4. If already registered, enter user name and password
5. If you are a new member, several screens will pop up including
· Member Services Function
· Anthem Member Services On Line Access Agreement
· Member Registration Screen (follow the prompts)
6. Once registered as a new member or signed on as an existing member, the MyAnthem screen will appear
7. At the top of MyAnthem screen select the 360 tab
8. In the Stay Healthy portion of this screen, select MyHealth Assessment and follow the prompts.
When you complete your Health Risk Assessment (HRA) you will be mailed a reward certificate in the amount of $25. The reward certificate will contain a PIN number and URL that you can use to log on to the IncentOne site to redeem your rewards. Your gift card will then be mailed to you.
