HOW TO ORDER A TRANSCRIPT

FOR ALL STUDENTS EXCEPT TEACHER ENHANCEMENT COURSES

To help protect your privacy, please read carefully.

IMPORTANT NOTE FOR STUDENTS ATTENDING PRIOR TO FALL 1988:
Due to a large archiving process many of these transcripts are currently unavailable. Received requests are processed as able.

Federal law prohibits us from taking your transcript request by phone for security purposes. For your protection, we do not accept requests by email since email is not secure and we require a signature for release. To order copies of your unofficial* or official** CSM transcript(s), you may come to the Registrar's Office during business hours and complete the short request form OR we must receive a typed or written request from you by mail or fax. Our policy requires that the typed or written request MUST have your signature and should include the following information:

Please specify the type and number of transcript (official or unofficial) when ordering.

1)       Name used while attending CSM

2)       Contact information: Email, telephone number, or address

3)       CWID, social security number, or last 4 of SSN

4)       Dates of attendance (approximate)

5)       Date of birth

6)       Address to where you want the transcript(s) mailed or name of whom to pickup transcripts.
-- If you need the transcript sent overnight, you must provide the pre-paid overnight envelope(s). U.S. Mail or Fed-Ex are best. If the label(s) and envelope(s) are not provided, there will be a delay or it will be sent regular U.S.P.S. mail. If sending to an international address, please use the appropriate shipping forms for the company you are using or your request will be delayed or sent regular mail. NOTE: If a credit card is used for payment on your shipping label and the charge is declined and charged to CSM, a hold will be placed on your account until the amount charged to CSM is paid.

      Official transcripts MUST be mailed. Unofficial transcripts are printed on plain white paper and can be faxed.

7)       Special handling requirements (i.e. individually sealed envelopes with Registrar’s signature, several in one envelope, etc.). Note: If ordering more than one transcript, all will be sent in one envelope if not specified to be sealed individually.

8)       FEE: As of July 1, 2007, transcripts processed by the Registrar's Office (does NOT include Teacher Enhancement/Special Programs transcripts) no longer requires payment. Official transcripts sent via U.S. Postal Service first class or standard airmail do not require fees for postage. Special delivery services please refer to #6.

Requests in person can be fulfilled while you wait and a form is available at the Registrar's Office or you may send your request to the Registrar's Office at the address listed below. Your mailed request normally will be processed within 5 business days upon receipt. Exceptions to this are during the following heavy traffic times: end of semester week, first two weeks of a semester, and final dates for course withdrawal will delay processing for up to one week; the week before a semester begins and pre-registration week may delay processing by two weeks.
